Need to run a survey, obtain feedback, create a questionnaire or a quiz?

Are you looking for a quick and simple form? Check out Google Docs. If you already have a Google account it is very easy to create a form. Just click "New" and click "Form". It is a simple form but it can serve many purposes. Its main advantages are that it puts the results into a nice spreadsheet and you can embed the form in a website or blog.

If you would like to see an example, please have a look at the feedback form I set up recently for the 2009 BDent1 workshops. Question types you can use include text (short answer), paragraph (long answer), MCQs, checkboxes, choose from a list, and 1-10 scale.

Fusion Finds has created two very nice screencasts on using Google Docs forms. The first one shows you how to create a form. The second screencast shows how to edit and share the form, and how to access the data Please be patient, it may take some time for the screencasts to download. Occasionally you might have to hit the refresh button if the screencast doesn't start right away.
